Use this form to request information about whether MERP has a claim related to a deceased property owner.

What you may need
- Your contact information and company or firm name, if applicable
- The deceased owner's full name and date of death
- The deceased owner's Medicaid ID or Social Security number
- The complete address of the property
- Information about the person authorizing MERP to release claim information
- A signed authorization form, if you already have one
Important
You do not need an existing MERP case number or access code to submit this request. MERP staff completes the certification after reviewing the information and required authorization.
Your information is secure. Your session expires after a period of inactivity. Your request is not submitted to MERP until you complete the final Review and Submit step.
